Salary : $95,000-$120,000 per year

Haven Home Health seeking a Full-Time Home Health Physical Therapist (PT) to join our dedicated team and provide skilled therapy services to patients in the Boca Raton, Boynton Beach, and Delray Beach areas. If you are passionate about patient care and want to be part of a supportive home health team, we want to hear from you!

Position Details :

  • Schedule : Monday Friday, 8 : 30 AM 4 : 30 PM
  • Productivity : 32 points per week (full-time)
  • Patient Scheduling : Patients are grouped by location for efficiency

Requirements :

  • Active PT License in the state of Florida
  • 1 year of home health experience preferred
  • Lymphadema certification or experience is required
  • Must be willing to see patients in Boca Raton, Boynton Beach, and Delray Beach
  • Strong clinical skills and ability to work independently
  • Knowledge of Medicare guidelines and OASIS documentation preferred
  • Strong interpersonal, time management, and communication skills
  • Reliable transportation and valid drivers license
