Overview

We are seeking a compassionate and experienced Registered Nurse (Home Health) to join our client's expanding team in Glencoe, Illinois. This role is centered around delivering high-quality, patient-focused nursing care to individuals in their homes, promoting comfort, independence, and overall well-being. The ideal candidate will bring strong clinical expertise, excellent communication skills, and a commitment to exceptional patient care.

Key Responsibilities

Perform comprehensive patient assessments and deliver skilled nursing care in home settings

Develop, implement, and revise individualized care plans in collaboration with physicians and the interdisciplinary team

Educate patients and caregivers on disease management, medications, and post-care instructions

Monitor patient outcomes and adjust care plans to ensure optimal results

Maintain accurate and timely documentation in accordance with clinical and regulatory standards

Coordinate care and communicate effectively with the broader healthcare team

Schedule and Shift Details

Monday through Friday, day shift

One weekend per month

One to two on-call days per month

Two holidays per year

This is a Pay Per Visit position

Travel

Local travel required within the Glencoe area (travel reimbursement provided)

Qualifications

Required

Associate degree or higher in Nursing

Active Registered Nurse (RN) license in Illinois

Minimum of 1 year of professional RN experience (home health preferred)

Current BLS or ACLS certification through the American Heart Association

Valid driver's license, reliable transportation, and current auto insurance

Preferred

Previous experience in home health or community-based nursing

Strong clinical assessment and organizational skills
