Job Summary
The Advanced Practice Provider (APP) will serve as a key member of a multidisciplinary Tracheostomy and Ventilator Program within the Pediatric Heart Center. This high-risk population includes pediatric patients with congenital heart disease requiring tracheostomies and long-term mechanical ventilation. The APP will support inpatient and outpatient care, facilitate transitions between care settings, and help drive early interventions via remote monitoring. The Nurse Practitioner and Physician Assistant is responsible for the assessment, diagnosis, treatment, management, education, consultation, and health promotion and care coordination for patients and their families.
This dedicated role includes procedural responsibilities, family and provider education, and collaboration with pulmonary and cardiology specialists. The APP will also help grow the program through quality improvement efforts and protocol development.
