Position Title : Public Health Specialist
Reports To : Program Manager
Position Summary :
The Public Health Specialist engages individuals living with HIV or at high risk of infection, linking and retaining them in health care while supporting medication adherence. The role includes conducting confidential HIV, syphilis, and HCV testing, providing STI screenings, and coordinating services with Public Health Support Specialists and other team members. Services are provided across a 15-county region, with regular travel between the five Alliance for Positive Health offices. This position prioritizes candidates skilled in working with stigmatized and disenfranchised communities and knowledgeable about local resources.
Major Activities
Uphold best practices regarding confidentiality in all duties and communications, in accordance with Article 27-F, HIPAA, Alliance policies, and other applicable regulations.
Engage HIV+ and high-risk individuals who are out of care or inconsistently engaged. Use evidence-based techniques to support clients in improving health outcomes.
Identify and reduce barriers to care by facilitating access to needed services. When appropriate, conduct home visits or meet clients at community locations to enhance accessibility.
Provide confidential HIV / syphilis / HCV testing and STI screenings in targeted community venues following NYS DOH guidelines.
Collaborate with Public Health Support Specialists to link clients to medical care, treatment, and supportive services.
Distribute syringes when needed and connect clients with harm reduction services.
Integrate the Transtheoretical Model of Behavior Change into program delivery where applicable.
Lead multidisciplinary case conferences, coordinating with providers to promote engagement and retention in care. Advocate on behalf of clients to secure services that support independence and self-management.
Maintain complete and accurate documentation in line with NYS regulations and agency protocols. Track and report data as required.
Perform additional duties as assigned.
Qualifications
Associate’s degree in Human Services, Public Health, or related field (Bachelor’s preferred).
Relevant human services experience preferred.
Familiarity with target populations and service systems strongly preferred.
Valid driver’s license and reliable transportation required.
Ability to work flexible hours, including evenings and weekends.
Knowledge, Skills, and Abilities

Strong community engagement and liaison abilities.
Highly organized with strong attention to detail and time management skills.
Ability to maintain professional boundaries with clients and providers in line with agency and grant requirements.

Employment Classification : Non-exempt, full-time (37.5 hours per week)
Equipment Used : Standard office equipment (computer, phone, fax, copier, shredder) and motor vehicles.
