Job Title : Physical Therapy Assistant

License : Valid physical therapy assistant license required.

Must-Have : Previous skilled nursing facility (SNF) experience preferred.

Description :

Ensure the prescreen is filled out and attached upon submission.

Resume and license verification are also required.

If these items are not completed and attached, the candidate will be declined.

Candidates must complete a COVID-19 test 72 hours prior to the start date, with results issued 48 hours prior.

Review the credentialing checklist prior to submission.

Credentialing requirements include the following :

• State license verification.
• Resume.
• Three references.
• County criminal background check for the last seven years for all counties lived and worked.
• Federal criminal background check for the last seven years or more.
• Multi-state criminal search for the last seven years or more.
• Nationwide sex offender search for the last seven years or more.
• HHS / OIG search.
• GSA search.
• Social security number trace.
• One-step PPD or Quantiferon, or proof of positive PPD and chest X-ray.
• SAM search.
• Background check from the Virginia State Police.
• Mandated reporter form.
• Sworn statement.

Position urgency is classified as normal.

Holiday coverage is not required.
