Primary Responsibilities

Primary Care Services : Routine health maintenance and preventive care

Chronic Disease Management : Ongoing care for patients with chronic conditions

Acute Care : Management of urgent and acute illness

Patient Education : Health promotion and disease prevention counseling

Collaborative Care : Work with multidisciplinary healthcare team

Required Qualifications

Master's Degree in Nursing (MSN required)

Board Certified Nurse Practitioner (Adult / Family NP preferred)

Massachusetts Nursing License or ability to obtain

Primary care experience preferred

Strong communication and clinical skills
