Role Description
We are seeking a Hospice Nurse (Full-Time, On-Site) to join our team in Castro Valley, CA . The Hospice Nurse plays a vital role in delivering compassionate, patient-centered end-of-life care. Responsibilities include :
- Providing comprehensive clinical assessments and skilled nursing interventions in patients’ homes or residential care settings.
- Managing pain and symptoms effectively to promote comfort and dignity.
- Administering medications, performing skilled procedures, and monitoring patient status.
- Collaborating closely with the interdisciplinary team —including physicians, social workers, spiritual counselors, and home health aides—to ensure coordinated and continuous care.
- Educating and supporting patients, families, and caregivers on disease progression, comfort measures, and available hospice resources.
- Documenting all care in compliance with hospice regulations and organizational standards.

Qualifications
Current RN license in the State of CaliforniaProficiency in Palliative and End-of-Life CareExperience in Hospice and / or Home Health Nursing preferredStrong skills in symptom management , pain control, and patient educationExcellent communication and interpersonal skills to provide empathetic support to patients and familiesAbility to work both independently and collaboratively as part of a care teamCommitment to upholding hospice philosophy and regulatory standardsCurrent CPR / BLS certification and relevant hospice training or certifications (CHPN preferred, but not required)
