Schedule : Tuesday Saturday, 7 : 30 AM 4 : 00 PM (occasional extended hours)

Community LIFE is a Program of All-Inclusive Care for the Elderly (PACE), dedicated to helping older adults live independently in their homes while maintaining dignity, autonomy, and a high quality of life. Our interdisciplinary team of geriatric care professionals works collaboratively in specialized Day Centers to deliver comprehensive, person-centered care.

Position Summary :

We are seeking a compassionate and skilled Occupational Therapist to join our interdisciplinary team. In this role, you will provide therapy services to adults aged 55+ to help them maintain or regain independence in daily living activities. Your work will directly support our mission by enhancing participants mobility, comfort, and overall well-beingboth in our centers and through in-home visits.

Key Responsibilities :

  • Evaluate and treat participants to improve function in ADLs / IADLs, range of motion, strength, and pain management.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans.
  • Conduct in-home assessments and provide ongoing case management as needed.
  • Document care and progress in accordance with Community LIFE standards.

Qualifications :

  • Education : Bachelors degree (or higher) in Occupational Therapy from an accredited institution.
  • Licensure : Current Occupational Therapist license in Pennsylvania.
  • Experience : Minimum of 2 years working with older adults.
  • Other Requirements : Valid PA drivers license and reliable vehicle.
