MTM Pharmacist (Remote)

We are searching for a dedicated MTM (Medication Therapy Management) Pharmacist to join our dynamic healthcare team. Preferred Qualifications :

• PharmD degree from an accredited pharmacy program
• Experience in Medication Therapy Management (MTM)
• State pharmacy license in good standing
• Proficiency in utilizing MTM platforms and tools
• Strong analytical and problem-solving skills
• Excellent communication and collaboration abilities
• Certification in MTM services
• Familiarity with electronic health record (EHR) systems
• Dedication to ongoing professional development and continuous improvement
