Location : Rockland, NY / Mobile

Hours Per Week : 35

FLSA Status : Full-Time, Non-exempt

Salary Range : $50,000 - $60,000 annually

Key Responsibilities :

• Supervises the day-to-day functioning of the MOT.
• Oversee the Schedule of visits
• Ensure that all documentation is completed in a timely manner
• Provide supervision and direct service, which will include engagement, assessment, and accompanying the individual on behavioral health and medical appointments
• Support residential staff and family
• Work with hospital staff on discharge planning
• Assist in crisis prevention and response
• Additional Duties as assigned

The NYS Justice Center legislation defines anyone who has regular and substantial contact with any person with special needs as a “custodian” under the law and, therefore, a mandated reporter.
