New York State Licensed Care Manager

A company is looking for a Care Manager, Bilingual Spanish / English - Remote.

Key Responsibilities :

Advocates for and educates beneficiaries on services and self-management techniques

Conducts assessments to identify barriers and develops care plans aligned with treatment goals

Coordinates with multi-disciplinary health care teams to ensure person-centered care and maximize health outcomes

Required Qualifications :

NYS RN or LCSW / LMSW (any state)

Bilingual in English and Spanish

Experience managing member information in a shared network environment

Relevant work experience preferably as a Care Manager

Knowledge of community health practices for frail adults and cognitive impaired seniors
