Bachelor of Science in Nursing (BSN) degree, active registered nurse (RN) license and minimum 1+ years RN experience required. Applicants who do not meet these qualifications will not be considered.
An opportunity is available for an experienced Home Health Registered Nurse (RN) to deliver skilled nursing care to patients in their homes. This contract role requires a self-motivated and organized professional capable of working independently while upholding high clinical standards and clear patient communication.
Key Responsibilities :
- Conduct comprehensive OASIS assessments and develop individualized care plans.
- Provide skilled nursing services including wound care, medication administration, IV therapy, and management of chronic illnesses.
- Monitor patient progress, detect changes in condition, and communicate promptly with the care team and physicians.
- Educate patients and families on treatments, medications, disease processes, and preventive care.
- Maintain accurate documentation using Home Care Home Base (HCHB) electronic system.
- Coordinate care with therapists, social workers, and home health aides.
- Participate in case conferences, staff meetings, and ongoing education.
- Comply with all regulatory and agency standards for patient care and documentation.
Qualifications :
Current active RN license in good standing within the state of practice.Minimum of 1 year of recent home health nursing experience, with field experience preferred.Proficiency in Home Care Home Base (HCHB) and experience with OASIS assessments.Strong clinical judgment, critical thinking, and communication skills.Reliable transportation, valid drivers license, and auto insurance.Ability to manage a flexible field-based schedule independently.BSN preferred.Knowledge of Medicare and home health compliance standards is advantageous.Prior caseload management experience (2532 points weekly) preferred.Schedule & Setting :
Full-time 13-week contract.Monday through Friday, 8 : 00 AM 5 : 00 PM.Field-based care within an assigned territory involving daily travel to patient homes.Caseload expectation of 2532 points per week (35 patient visits daily).Mileage reimbursement provided.Benefits :
