We are looking for a reliable LPN (Licensed Practical Nurse) to manage a team of caring professionals that care for our very special residents in a professional and considerate manner. The hours for this role are primarily Monday – Friday, 11 : 00 a.m. – 7 : 00 p.m., with weekend and holiday on-call rotation.

Job Summary : Responsible for the direction of resident care in the skilled nursing setting.   Manages licensed and non-licensed clinical staff.   Consults with staff, physicians, management team on nursing problems and interpretation of facility policies to ensure resident needs are met.   Maintains performance improvement activities for Nursing Services and participates in QI activities.

Job Description

Responsibilities

• Provides the oversight of the nursing staff and residents on assigned unit 24 / 7, ensures adequate staffing and quality of care.
• Proper and consistent documentation in the resident record.
• Participates in all aspects of interdisciplinary committees as needed / assigned.
• Makes rounds on the nursing unit multiple times daily, ensuring the safe care of the residents.
• Treats residents and their families with respect and dignity.
• Knowledge of CMI quality measures.
• Identifies and responds to residents change in condition as they occur.
• Completes medication passes as needed or directed.
• Coordinates and directs resident care through communication and care planning.
• Conducts staff meetings, including daily huddle and report.

Skills

Proven experience as LPN or similar role.

Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team.

Understanding of confidentiality obligations and nursing best practices.

Excellent communication and interpersonal skills.

Compassionate and able to handle stress.

Successful completion of LPN license program is a must.
