Travel Physical Therapist (PT) – Long Term Care (LTC)

Location : Snyder, TX

Start Date : 10 / 27 / 2025 | Duration : 13 Weeks

Shift : Days | 8 hours / day | 40 hours / week

Weekends : None | On Call : None

Position Summary :

The Physical Therapist (PT) provides comprehensive physical therapy services to residents in a long-term care setting. The PT assesses, diagnoses, and treats movement dysfunction to help patients restore function, relieve pain, and improve mobility and quality of life.

Essential Functions & Responsibilities :

Perform patient evaluations, including history, systems review, and selection of appropriate tests and measures.

Develop and implement individualized treatment plans in collaboration with the interdisciplinary team.

Conduct ongoing reassessment to ensure optimal outcomes.

Initiate and manage discharge planning beginning with the first visit.

Supervise and delegate tasks to assistants and students per state law and facility policy.

Complete timely and accurate documentation per policy and regulatory standards.

Provide education to patients, families, and caregivers to support rehabilitation goals.

Participate in professional growth, continuing education, and quality improvement activities.

Maintain a safe and therapeutic environment for all patients and staff.

Requirements :

Minimum 2 years of PT experience in a Long Term Care (LTC) setting – Required

Texas or Compact PT License – Required

BLS (AHA or Red Cross) – Preferred
