Type : 13-Week Contract (Travelers & Locals Welcome)

Duration : 13 Weeks

Shift : MondayFriday, with rotating on-call

Job Description :

Sunbelt Staffing is seeking a Home Health RN for a 13-week contract in the Phoenix area. This position involves independent field nursing, caring for patients in their homes. Must be proficient in OASIS assessments and experienced with Homecare Homebase. Strong communication and time management skills are essential.

Key Responsibilities :

Manage a caseload of home health patients

Complete OASIS Start of Care, Recertification, and Discharge visits

Use Homecare Homebase for documentation

Coordinate with physicians and the care team

Educate and empower patients in managing chronic conditions

Cover on-call as needed

Qualifications :

Arizona RN license (active and in good standing)

2+ years of home health experience

Strong knowledge of OASIS and Homecare Homebase

Current BLS from AHA

Drivers license, car insurance, and dependable vehicle

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
