The Department of Developmental Services (DDS) is seeking a motivated and experienced human service professional for the role of a Transition Services Coordinator (Human Services Coordinator I) located in New Bedford, Mass. The Transition Services Coordinator coordinates, integrates, and monitors services provided to assigned clients; acts as advocate, provides information and referral services to transitional age youth and families; acts as liaison and support to transitional age youth and families with other community service providers.
Duties and Responsibilities :
- Makes necessary referrals to address issues and requests to the most appropriate and efficient provider program(s). Screen telephone requests for services to determine the nature of request or problem.
- Acts as liaison for transitional age youth services (17.5 to 22) to families, family service providers, Special Education Departments (out-of-district and home care placements included), DCF, and other relevant state agencies.
- The incumbent will work effectively, cooperatively, and in a timely manner with individuals, their families and collaterals from state, public, and private entities towards common goals that increase the capacity and capabilities of individuals and their families.
- Awareness of and familiarity with legislation on the state and federal level as it pertains to transitional age youth and families.
- Initiates, develops and maintains positive relationships with local school district superintendents and special education directors.
- Identifies those transitional age youth (17.5-22) determined by schools and / or other transitional age youth service provider to be at risk for out-of-home placements and plans for services to support placement at home.
- Provides technical assistance to families, school personnel.
Preferred Qualifications :
Familiarity with Microsoft programs and Home and Community Services Information System (HICSIS).Experience working with individuals with Intellectual DisabilitiesExperience working with transitional age youthAbility to develop and maintain respectful, caring, patient, ethical, and empowering relationships with families and individuals supported by the department.Excellent organizational and communication skillsSkills using communication styles and / or augmentative communication systemsWork Schedule :
Full-time – 37.5 hour weekly position, Monday through Friday – 9 : 00 a.m. to 5 : 00 p.m. Flexibility with work schedule is required.
