Shift : Monday - Friday | 8 : 30 AM - 5 : 00 PM | 40 Hours / Week

Contract Duration : 2+ Months

Compensation : $1,500 - $2,000 per week

Job Summary :

A well-established healthcare organization is seeking a dedicated Registered Nurse Case Manager to provide comprehensive case management and nursing support during the prenatal and postpartum periods. The ideal candidate will offer compassionate, evidence-based care, perform client screenings, provide education, and ensure coordinated access to community resources.

Key Responsibilities :

• Deliver equitable nursing case management services for prenatal, postpartum, and infant care populations.
• Conduct screenings for Social Determinants of Health (SDOH) , perinatal depression, and other prenatal / postpartum risk factors at admission and as needed.
• Document all assessments and interactions in the Electronic Medical Record (EMR) in a timely and accurate manner.
• Offer client education and guidance in line with current perinatal care standards .
• Provide short-term interventions , emotional support, and connect clients with appropriate community resources.
• Develop personalized care plans and track client progress through measurable goals.
• Respond to all communication (email, phone, or text) within one business day.
• Conduct postpartum home visits within 30 days of delivery as needed.
• Maintain accurate program data and submit monthly statistics to supervisor by the 5th of each month.
• Participate in required virtual meetings, workshops, and training sessions.
