Primary Care Physician – PACE Program
Our client is seeking a dedicated Primary Care Physician to provide comprehensive, compassionate care for participants enrolled in their PACE (Program of All-Inclusive Care for the Elderly) program. This position plays a key role in promoting participants’ health and independence by coordinating medical care and collaborating with an interdisciplinary team (IDT) to ensure the highest quality of care.
Position Highlights :
- Deliver comprehensive, coordinated medical care guided by the PACE benefit model.
- Conduct initial, semiannual, annual, and periodic participant assessments.
- Partner with the interdisciplinary team to develop, review, and implement individualized care plans.
- Oversee diagnostic testing, medication management, and appropriate specialist referrals.
- Provide follow-up care and collaborate with participants’ families and support networks.
- Participate in team meetings, care conferences, and family discussions as needed.
- Coordinate care for hospitalized and nursing home participants, ensuring smooth transitions and adherence to care plans.
- Participate in quality assurance and performance improvement programs (QAPI).
- Document all care in eClinicalWorks (EMR) and maintain compliance with all policies, procedures, and HIPAA regulations.
Additional Responsibilities :
Offer medical guidance and leadership to the IDT and collaborate with clinic leadership on site-level medical operations.Participate in after-hours telephonic coverage on a rotating basis.Contribute to committees and initiatives such as infection control, ethics, and quality improvement.Maintain a team-oriented approach to patient care and collaboration.Qualifications :
Board Certified or Board Eligible Physician in Internal Medicine or Family Medicine.Experience with geriatric or complex care populations preferred.Strong communication, collaboration, and documentation skills.Commitment to patient-centered, interdisciplinary care.EMR : eClinicalWorks
Supervision : This role does not have oversight of APP staff (Regional Medical Officer provides oversight).
