\nStartDate : 10 / 19 / 2025 \nAvailable Shifts : 9.5 D \nPay Rate : $2013.18 - $2205.93 \n

\n2+ years of experience. 50-mile radius rule. No more than a 3-month work history gap in the last year. Current ME Licensure as a PT, BLS and valid Driver’s license (with vehicle available to work covered by liability insurance as specified by agency policy) required. Must have at least 1 year of Home Health experience, possess strong interpersonal skills, flexibility, and the ability to build and maintain positive and genuine client rapport.\n

Physical Therapist, Home Health\nExperience : 2 years\nLicenses : PT- ME\nCertifications : BLS-AHA(Copy needed)\nSSN Required\nDOB Required\nWork History : Should not be more than 90 days of gap in last 1 year\nReferences : 1 Reference in entire work history\n\nEntry-Level Degree in Physical Therapy

Must have 1+ years of HH experience

Valid Driver’s license (with vehicle available to work covered by liability insurance as specified by agency policy) required\n\n\n\n
