Indiana Licensed Admission Nurse

A company is looking for an Admission & Discharge Registered Nurse.

Key Responsibilities

Conduct initial evaluations to determine patient needs and initiate care plans

Collaborate with teams to ensure timely admissions and regulatory compliance

Coordinate care with other agencies and perform discharge visits as required

Required Qualifications, Training, and Education

State of Indiana or multistate RN license

Graduate of an accredited nursing school; BSN preferred

Home care experience preferred

Experience with medically fragile clients is beneficial

Ability to exercise initiative and independent judgment
