About The Position

Responsibilities

Evaluate, plan, and implement individualized treatment programs for residents.

Collaborate with interdisciplinary teams to ensure quality patient outcomes.

Maintain accurate documentation in accordance with facility standards.

Educate patients, families, and caregivers on therapeutic exercises and care plans.

Job Requirements

Active Texas PT License (or eligibility).

Graduation from an accredited Physical Therapy program.

Prior LTC or SNF experience preferred (new grads encouraged to apply).

Strong communication, compassion, and teamwork skills.
