Position Summary
The Case Manager will provide in home skills training and case management services on an individual basis to children / adolescents and their families who are working with the Family Health Center. The Case Manager can also provide these services in a group setting within the office or community using evidence-based curriculum.
Essential Duties and Responsibilities
The essential functions may include, but are not limited to the following :
1. Conduct assessments of assigned youth and their families within the timeline identified by policy.
2. Develop and complete Person-Centered Treatment Plans (PCTP) and treatment plan reviews with youth and their families within the timeline identified by policy.
3. Provide services to youth and their families as outlined in the PCTP including case management and individual skills training.
4. Enter progress notes, complete State Registration and CANSA documents, maintain schedule in EPIC, and complete billing documentation within the timeline identified by policy.
5.Participate in team meetings, communicate and cooperate with both internal and external team members regarding tasks, and contribute actively to the team's efforts to ensure the youth and their family's needs are met.
6. Meet with youth and families based upon the level of need of the CANSA (Child and Adolescent Needs and Strengths Assessment) and determined by the team.
7. Maintain knowledge of Medicaid Rehabilitation Option (MRO) guidelines regarding provision of services, documentation requirements, qualifying diagnoses, and level of need issues.
8. Follow expectations set forth by the Family Health Center in regard to training, productivity, policy, and documentation.
