Role Description

This is a full-time on-site role for a Registered Nurse Case Manager located in Florence, AL. The Registered Nurse Case Manager will be responsible for assessing patient needs, developing and implementing care plans, coordinating with other healthcare professionals, and providing direct patient care. The role also involves monitoring patient progress, managing medication, and supporting the emotional and spiritual needs of patients and their families.

Qualifications

• Nursing skills : Registered Nursing License, patient assessment, care planning, direct patient care
• Coordination and Communication : Coordination with healthcare professionals, excellent communication skills
• Emotional and Spiritual Support : Ability to provide emotional and spiritual support to patients and families
• Relevant experience in hospice or palliative care
• Ability to work independently and on-site
• Compassionate and empathetic demeanor
