Position Overview :

The Nurse Practitioner (NP) provides Hepatitis C Care, HIV Care, and advanced sexual health care, and takes an active leadership role in clinical service provision with a focus on health care for people who use drugs and those experiencing homelessness.

The NP works under a clinical scope of practice with the Senior Director of Clinical Services and Medical Director. Clinical services are provided across two sites : at the 6th Street Center in SoMa, and at Strut in the Castro neighborhood. This is a full-time position working four (4) 10-hour, rotating shifts per week, including Saturdays.

Essential Duties :

• Clinical Services : Serves as the primary interface with clients seeking screening for sexual health services, Hepatitis C treatment, HIV Pre-Exposure Prophylaxis (PrEP) and non-Occupational Post Exposure Prophylaxis (nPEP) services, HIV Rapid initiation and re-engagement to care, wound care, and medication assisted treatment and counseling for opioid use. Provides direct clinical services as defined by Magnet's protocols and treatment guidelines including obtaining medical history and physical, specimen collection, high-volume phlebotomy, administration of treatments, performing point of care lab tests, management of Hepatitis C, PrEP and nPEP care, HIV care, buprenorphine management, client counseling, and education. Works with Magnet staff and other volunteers to insure quality of care. Provides referrals as appropriate to outside providers. Serves as a clinical resource for a team of Registered Nurses (RNs).
• Hepatitis C Care : Provides direct medical care to individuals living with HepC. Will be responsible for delivering HepC diagnosis to individuals newly infected with HepC, provide initiation of DAA therapy to newly diagnosed individuals ready to start treatment, review current DAA regimens for individuals previously diagnosed with HepC seeking to re-engage in medical care, interpret and review laboratory findings with clients, and support primary care needs during treatment.
• HIV Care : Provides direct medical care to individuals living with HIV / AIDS. Will be responsible for delivering HIV diagnosis to individuals newly infected with HIV, provide immediate initiation of ART therapy to newly diagnosed individuals ready to start treatment, review current ART regimens for individuals previously diagnosed with HIV / AIDS seeking to re-engage in medical care and make any necessary ART regimen adjustments, interpret and review laboratory findings with clients and adjusting ART regimens based on lab findings when appropriate.
• Administration : Serves as a clinic leader and assists in the coordination of clinic operations, flow, and client care. Completes documentation of reports and clinical services provided with a high level of detail and thoroughness. Provides other professional or administrative duties as required.
• Public Health Reporting : Ensures Confidential Morbidity Reports are submitted to the Department of Public Health and address any submission issues.
• Laboratory Testing Personnel : Perform moderate complexity lab testing, waived lab testing, and Provider Performed Microscopy per the laboratory guidelines under the supervision of the Laboratory Director.

This position will have access to protected health information (PHI) and may only access, use or disclose the minimum information necessary to perform their designated role on behalf of San Francisco AIDS Foundation, regardless of the extent of access provided.

Work Environment :

This position is primarily located at 117 6th Street and 470 Castro Street, San Francisco, however it may occasionally require traveling to other office locations or off-site meetings as required by the job. Role may include evening and weekend coverage.
