Responsibilities
Complete initial and ongoing comprehensive assessments of patients and familiesInitiate patient plan of care and reevaluate as necessaryUtilize health assessment data to determine nursing diagnosisAdminister medication and treatment as prescribed by the physician in the plan of careCounsel the patient and family in meeting nursing and related needsComplete and maintain accurate clinical notesCommunicate with the physician and participate in interdisciplinary group meetings to meet the medical and emotional needs of the patient and family / caregiversEnsure compliance with all state and federal legal and regulatory requirementsAdditional responsibilities as assignedQualifications
Graduate of an accredited nursing school with current licensure to practice in state of operation1 year nursing experience required, 3+ years preferredExperience in hospice or a similar setting preferredKnowledge of the hospice philosophy of careCommitment to clinical and documentation excellence
