About The Position
Position Summary
The Physical Therapist is responsible for evaluating, diagnosing, and treating patients with physical impairments that impact their mobility and daily functioning. The therapist develops individualized treatment plans with patient-centered goals to reduce pain, restore function, and prevent disability, while also working proactively with patients to prevent loss of mobility. Care will be delivered in compliance with physician orders, regulatory standards, and best practices in long-term care (LTC) settings.
Essential Functions and Responsibilities
Perform comprehensive patient evaluations by obtaining medical history, conducting system reviews, and administering specific diagnostic tests and measures.
Develop individualized plans of care that address identified impairments and align with physician orders and patient goals.
Deliver and modify therapeutic interventions aimed at improving mobility, restoring function, and preventing disability.
Conduct ongoing reassessments of patients to monitor progress and adjust treatment plans as necessary.
Initiate and manage discharge planning starting from the first visit and throughout the course of treatment.
Supervise and delegate tasks to ancillary staff and students in compliance with state laws and departmental policies.
Document all patient interactions, assessments, treatment plans, and outcomes accurately, clearly, and in a timely manner in accordance with accreditation and insurance standards.
Communicate effectively with patients, families, caregivers, and interdisciplinary team members using the AIDET framework to maximize patient care outcomes.
Educate patients and caregivers on treatment techniques, safe mobility practices, and home exercise programs to promote independence.
Actively participate in professional development, quality improvement initiatives, and ongoing training opportunities.
Ensure a safe environment for patients, staff, and visitors by following established safety standards and infection control policies.
Practice within the scope of licensure as defined by the state’s practice act.
Perform other related duties as assigned to support departmental and organizational goals.

Job Requirements
Qualifications
Experience : Minimum 2 years of Physical Therapy experience in a Long-Term Care (LTC) setting required.
Licensure : Active Texas Physical Therapy license or Compact State license required.
Certifications : BLS certification preferred (if applicable to facility policy).
Skills : Strong clinical assessment, documentation, and communication skills; ability to work collaboratively in a multidisciplinary team environment.
