Join our team as a Home Health Physical Therapist, delivering vital care to patients in their homes. This role requires recent home health experience and proficiency in HCHB / OASIS documentation.

Key Responsibilities :

Provide physical therapy in a home health setting.

Complete Homecare Homebase documentation on

site.

Attend weekly in

person office meetings.

Offer flexible scheduling to meet patient needs.

Requirements :

Valid PT License

BLS Certification

2+ years of recent Physical Therapist experience

Recent home health experience with HCHB / OASIS

Reside within 30 minutes of the territory or secure nearby housing

Contract Details :

week contract, 40 hours / week

No on

call or overtime

Mileage reimbursement

Additional Information :

3 days of orientation
