General Summary
Provides comprehensive family-oriented nursing services to individuals in the home setting by use of the nursing process in accordance with physician orders, the established plan of care, corporate policy and procedures, and the Nurse Practice Act.

Essential Functions
- Participates in the development of a written plan of care in accordance with the patient's diagnosis, patient and family needs, physician's orders, and the corporate policies and procedures.
- Provides selected nursing procedures and treatments to patients as outlined in the plan of care and assigned by the primary nurse.
- Notifies physician and other care agencies of changes in patient's condition and care needs and obtains new orders, as needed.
- Assists with coordination of patient care through dialogue with other disciplines involved with the patient and documents same.
- Participates in patient care conferences.
- Documents observations and intervention in the patient's clinical record in a timely, legible, and accurate manner.
- Functions as a member of an interdisciplinary team, encouraging and maintaining open communications at all times.
- Fosters a climate of mutual respect and teamwork among co-workers and management.
- Assist in the orientation of new staff.
- Submits patient visit schedule to Scheduler and communicates significant changes to Primary Nurse.
- Functions in a cost-effective manner in regard to utilization of time, mileage, supplies, etc.
- Meets or exceeds established productivity and quality assurance standards.
- Seeks consultation, guidance and direction from professional or administrative staff, as needed.
- Informs manager of perceived problem areas related to patient care, corporate management, or community relations.
- Keeps clinical skills current with nursing practice norms.
- Exhibits a comprehensive knowledge of, commitment to and demonstrated ability in the practice of home health nursing.
- Exercises sound judgment and works effectively under pressure.
- Demonstrates a working knowledge of skills and services provided by each member of the multi-disciplinary team and the ability to relate these to identified patient needs.
- Acquires and utilizes knowledge of community resources.
- Elicits feedback from appropriate sources regarding the quality of services provided.
- Demonstrates a professional attitude, sound judgment and continuity of interactions with physicians, other health care providers and community agencies.
- Represents the VNA at community activities, as delegated.
- Promotes a positive corporate image.
