RN Case Manager

to coordinate patient care, manage utilization, and support discharge planning within an acute hospital setting. This role involves working closely with interdisciplinary teams to ensure smooth transitions of care and adherence to quality standards.

✅ Requirements : Experience :

3–4 years recent case management, utilization management, or discharge planning (teaching hospital preferred)

Certs :

BLS, BSN, Active CA RN License

Skills :

InterQual, discharge planning, concurrent review, and care coordination

Schedule :

4x10-hour days (Mon–Fri, variable day off) | No weekends

Start Date : 12 / 01 / 2025
