Graduate of an accredited school of professional nursingCurrent license as a Registered Nurse (RN) with the State of MNCPR certificationDemonstrated knowledge of safety and infection control practicesExcellent organizational skillsExtraordinary communication skillsAbility to use technology solutions for scheduling, charting, training and reviewingBe involved and team focused. We are looking for someone hungry to help find solutions to challenges presented each day.Experience in home care (preferred, not required).Experience developing training programs (preferred, not required).What you will be doing :
Performing comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.Developing an individualized plan of care for each client that incorporates analysis of data and current scientific findings.Providing interventions with a focus on achieving realistic client outcomes.Reporting changes to update the plan of care to reflect progress towards goals and outcomes.Administering medications as needed.Managing and delegating tasks to other nurses and caregivers.Maintaining client records showing systematic assessments, planning interventions, and evaluations.Initiating referrals as needed to other health team members.First shift introductions of caregivers to clients.Train and engage caregivers to deliver quality, consistent care.Operate orientations and training programs for staff.Manage QAPI program.Schedule :
Full-Time or Part-TimeMonday - FridayOn Call Rotation for triage, client intros, orientations, assessments (as needed)Flexible work from home options available.
