This full-time role requires Monday through Friday hours from 8 AM to 5 PM, and you will be onsite at an assigned hospital daily from 9 AM to 3 PM, working remotely for the remaining hours. Your role as an Acute Care Nursing Manager is pivotal in ensuring positive patient outcomes and maintaining a high standard of care. You will advocate for patients as they navigate complex healthcare systems and external providers.

As an Acute Care Manager, Complex Care (RN), you will work closely with a dedicated Complex Care Team, utilizing available resources to develop effective care plans that emphasize coordinated care. Building strong relationships with patients, families, and other healthcare professionals will be critical as you work to facilitate safe and timely discharges to the most appropriate level of care.

Key Responsibilities :

  • Maintain a daily presence at the assigned hospital during core hours, ensuring balanced caseload management.
  • Identify opportunities for optimal healthcare resource allocation and assess inpatient vs. observation status.
  • Conduct safety risk assessments and collaborate on care plans to enhance patient self-management and knowledge.
  • Partner with the Complex Care and Clinical Strategy Teams for effective implementation of acute care procedures.
  • Facilitate communication between patients and care managers, ensuring a seamless transition from hospital to home or further care facilities.
  • Document patient progress accurately and participate in multidisciplinary care discussions as needed.
  • Perform comprehensive screenings to identify social determinants of health issues and refer appropriate services.
  • Assist patients in accessing community resources and plan follow-up care with primary care providers.

Qualifications :

  • Bachelor's Degree in Nursing (BSN) or related clinical field preferred.
  • Active Registered Nurse (RN) license in the state of employment required.
  • Minimum of two years of clinical experience with one year in utilization review or case management strongly preferred.
  • Basic Life Support (BLS) certification within the first 90 days of employment.
  • Certification as a Case Manager preferred.
