Maryland Licensed Social Worker

A company is looking for a Social Worker Care Manager (Hybrid) to service patients in the Frederick, MD area.

Key Responsibilities

Support mental and physical health care for an assigned patient caseload

Screen and assess patients for mental health and substance abuse disorders, facilitating engagement and follow-up care

Document patient progress and treatment recommendations in EHR and other required systems

Required Qualifications

Minimum of MSW

Preferred Licensure or provisionally licensed

Behavioral health experience and knowledge

Must be a licensed driver with an insured automobile in good working order

Comfortable utilizing Excel and Word for assessments and productivity tracking
