Description :

This position involves case management in the ambulatory healthcare setting.

The RN Case Manager participates as a crucial member of the care team, which includes providers, licensed practical nurses, medical assistants, and patient panels.

Pre-visit planning for scheduled patients is an integral part of the role.

The RN communicates patient needs and recommended treatments to the care team.

Maintaining and updating the patient panel is required.

The RN identifies, creates, and maintains panels of high-risk subpopulations for care management.

Tracking performance measures monthly and sharing this information with the team, staff, and patients is essential.

Participation in ongoing quality improvement activities using the Model for Improvement is expected.

The RN contacts patients for follow-up on diagnostic tests, referrals, appointments, or medical follow-up as appropriate.

Scheduling nursing visits for follow-up care is a key responsibility.

Tracking and following up on referrals is also required.

Providing patient education and self-management support is an important part of the role.

Triage of patients on the team's panels who call or walk in and directing them appropriately is necessary.

Documenting observations, assessments, nursing interventions, treatments, and referrals according to documentation standards is mandated.

The RN may provide direct nursing care as needed by the team.
