RN - Nurse Case Manager - Local Contract

WCS Healthcare is partnering with a reputable assisted living organization seeking a qualified RN to work a Monday - Friday 9am-5pm schedule as a Nurse Case Manager conducting assessments for Long Term Care services in Manhattan.\n\nJob Function\n

• Day Shift.\n
• 9am - 5pm.\n
• Collaborate with members and their families to maintain the most independent living situation possible.\n
• Ensure consistent care across the healthcare continuum by assessing and closely monitoring members' needs and status.\n
• Authorize covered services and coordinate care across different payers.\n
• Communicate and collaborate with members, family members, caregivers, primary care practitioners, and the interdisciplinary team.\n\nJob Requirements\n
• NYS RN License.\n
• Bachelor of Science in Nursing (preferred).\n
• Associate's Degree in Nursing required.\n
• Minimum of 3 years of RN experience.\n
• Bilingual in Spanish or Chinese (preferred).\n
• Must have Care Management, UAS, or Managed Long-Term Care experience.\n
