Benefits may vary by employment status Responsibilities As a RN Case Manager You will : Assess / monitor physical, emotional, and psychological needs of patients Create hospice care plans that align with the patient's wishes and goals Direct nursing care : administering medications, treatments, and interventions Provide pain and symptom management Educate and support the patient’s family and caregivers Collaborate with interdisciplinary team Maintain accurate and timely documentation Participate in on-call rotation as required by local branch Qualifications Graduate of an accredited nursing school with current licensure to practice in state of operation One year nursing experience required , 3 years preferred Experience in hospice or a similar setting preferred Knowledge of the hospice philosophy of care Commitment to clinical and documentation excellence Must be a licensed driver with an automobile that is insured in accordance with state / or organization requirements and is in good working order About Our Line Of Business At CompassionCare Hospice, our primary ambition is to bring first-class care to the warmest and most serene environment possible : home.
