This role applies clinical knowledge, critical thinking, and utilization review principles to promote timely, safe, and appropriate transitions of care. The Case Manager serves as a key member of the multidisciplinary team, advocating for patient needs, collaborating with providers, and ensuring optimal outcomes through effective planning and communication. Requirements Education : Graduate of an accredited school of nursing. Associates Degree required; Bachelors Degree in Nursing (BSN) preferred or actively pursuing. Experience : Minimum of 3 years of recent clinical nursing experience required. Previous experience in case management, discharge planning, utilization review, or care coordination strongly preferred. Familiarity with community resources and post-acute care services beneficial. Licensure : Current and active Registered Nurse (RN) license in the State of Florida required. Certifications : Basic Life Support (BLS) American Heart Association required. Case Management certification (ACM, CCM, NACCM, or CMCN) preferred. Skills and Abilities : Strong assessment, organizational, and critical-thinking skills. Excellent communication and interpersonal abilities to collaborate with patients, families, and interdisciplinary teams. Knowledge of medical necessity criteria, insurance authorization processes, and hospital discharge planning standards.
