CNA Coordinator

We're looking for a CNA Coordinator to join our compassionate and mission-driven team. In this critical role, you will coordinate and schedule CNA visits based on patient care plans, ensure compliance with regulatory requirements, and help create an extraordinary care experience for our patients and their families.

This position is ideal for someone who thrives in a fast-paced environment, enjoys problem-solving, and has a strong background in healthcare operations, particularly in home health or hospice care .

Key Responsibilities :

Scheduling & Coordination

• Maintain daily CNA schedules, accounting for staff availability, vacations, and absences
• Schedule initial CNA visits and supervisory visits in alignment with care plans
• Assign consistent CNAs to patients to ensure continuity of care
• Coordinate with families, patients, and clinicians to ensure visits meet patient needs and expectations
• Communicate appointment details to patients and healthcare facilities
