Additional Details

Proof of 2 immunizations, positive titer or Declination accepted.

Must be one of the 3 mask fit types. N95- 3M 1860, 1860s or 1870+. Must be completed within 1 year prior to start. Updated annually.

Proof of 3 immunizations, positive titer or Declination accepted.

Flu Season dates to be communicated each year. Only Medical / Religious exemptions will be accepted with a physician’s signature or written signed statement from traveler.

Proof of negative Blood test or Skin test as described below.

Statement signed and dated within 1 year of start stating traveler is able to work without restrictions.

Proof of vaccination within past 10 years or Declination accepted.
