The Part-Time Nurse Practitioner (NP) will conduct home health risk assessments within Fairfield County, OH. The average home health risk assessment will take approximately 45 minutes, and the nature of the visit may vary, potentially including an annual health assessment or a post-discharge visit. The home health risk assessment will be conducted on Medicare Advantage plan members residing in Lancaster, OH,and the surrounding Fairfield County, OH.Fairfield County, OH,includes locations like Columbus, Violet Township, and Reynoldsburg.Candidates could be scheduled in any one of these locations during an individual shift.

Responsibilities :

• Conduct annual home health risk assessments for a Medicare Advantage health plan
• Evaluate prior and current medical history, medical conditions and medication history
• Assess for Gaps in Care and provide recommendations to the primary care physician for gap closure
• Provide education on medical conditions and medications
