Compensation : Pay per visit and processed bi-weekly.

Shift Type : PRN, Monday - Friday, 8 AM to 4 PM (flexible), no weekends.

Summary :

The Physical Therapist ('PT') is responsible for evaluating / assessing the needs of our patients and formulating treatment plans in the home care environment. A PT provides therapy services, including the creation and administration of care plans, as well as oversees physical therapy assistants. A PT must work collaboratively across the internal interdisciplinary care team, physicians, and other stakeholders in our patient's care.
