Description

• Perform home visits and assess patient's health problems and needs as well develop and implement nursing care plan.
• Implements care by setting priorities based on the client care plan and immediate client need.
• Document normal and abnormal conditions and reports accurately and timely to doctor.
• Evaluates the response of clients to nursing interventions and revises care plans with involved department personnel as needed.
• Participates in teaching client, family members and / or significant others as appropriate.
• Oversee home health aides assigned to clients.
• Participates in evaluation of clients in preparation for discharge.
• Document data related to patients' care including assessment results, intervention, medications, patient responses or treatment changes.
• Monitor, record and report symptoms and changes in patients' condition including but not limited to vital signs.
• Consult and coordinate with health care team members to assess, plan, implement and evaluate patient care plans.
• Discuss illnesses and treatments with patients and family members.
• Monitor all aspects of patient care including diet and physical activity.
• Supports and abides by the mission, values, and policies of CHCS in all activities.
• On-going compliance with CHCS educational requirements.
• Arrives on time and ready to work.
• All other duties as required.

Requirements

Current Registered Nurse in the State of Kansas

BLS Certified

Valid Kansas Driver's License
