- Manage after-hours patient and family phone calls, assessing the need for services such as assessments, symptom management, and care plan updates, while delegating nursing visits to appropriate staff members.\n
- Gather information on physical, psychological, social, and spiritual factors impacting patient and family needs, initiating interventions by other interdisciplinary team members as necessary.\n
- Communicate with attending physicians and interdisciplinary staff to coordinate optimal care.\n
- Perform clinical pronouncement of death and provide family support during this sensitive time.\n
- Maintain accurate clinical records by timely preparing care plans and documenting clinical notes and progress.\n\nQualifications : \n
- Graduate from an accredited registered nursing program.\n
- Current license to practice in the state as a Registered Nurse.\n
- A minimum of two years of diverse professional nursing experience.\n
- Proven skills in nursing practices and clinical judgment.\n
- Ability to manage family emotional stress and respect individual lifestyles.\n
- Current CPR Certification is required.\n
