• Manage after-hours patient and family phone calls, assessing the need for services such as assessments, symptom management, and care plan updates, while delegating nursing visits to appropriate staff members.\n
• Gather information on physical, psychological, social, and spiritual factors impacting patient and family needs, initiating interventions by other interdisciplinary team members as necessary.\n
• Communicate with attending physicians and interdisciplinary staff to coordinate optimal care.\n
• Perform clinical pronouncement of death and provide family support during this sensitive time.\n
• Maintain accurate clinical records by timely preparing care plans and documenting clinical notes and progress.\n\nQualifications : \n
• Graduate from an accredited registered nursing program.\n
• Current license to practice in the state as a Registered Nurse.\n
• A minimum of two years of diverse professional nursing experience.\n
• Proven skills in nursing practices and clinical judgment.\n
• Ability to manage family emotional stress and respect individual lifestyles.\n
• Current CPR Certification is required.\n
