These values - integrity, patient-centered, respect, accountability, and compassion - must guide what we do, as individuals and professionals, every day.\n\nThe Registered Nurse Case Manager is responsible for admitting patients for home care services, developing the patient's plan of care, coordinating with the patient's provider and other disciplines for the provision of services, and ensuring the safe delivery of patient care.\n\nEEO / AA / Disability / Veteran\n\nResponsibilities\n

• 1. Assesses / reassesses patients and establishes plans of care that are patient specific.\n
• 2. Collaborates with providers, other disciplines, and communicates all changes promptly.\n
• 3. Assesses payment sources and is responsible for ensuring ongoing authorizations as necessary.\n
• 4. Educates and instructs patients / caregivers in their ongoing medical maintenance. Begins discharge planning on admission.\n
• 5. Completes all documentation within 24 hours. Must include changes in condition, follow-up with provider or other disciplines with supporting documentation.\n
• 6. Evaluation of patient progress and prompt action\n
• 7. Oversight of LPNs and Home Health Aides.\n
• 8. Schedules patients independently according to the individual plan of care.\n
• 9. Attends all staff meetings, inservices, other required meetings. Compliance with all mandatory requirements.\n
• 10. Follows policies and procedures.\n
• 11. Thorough and accurate OASIS documentation.\n
• 12. Coordinates and manages the care of patients receiving services in their home.\n
• 12.1 Participates in case management conferences and documents per guidelines.\n
• 13. May perform other duties as assigned.\n\nQualifications\n\nEDUCATION\n\nNursing Diploma or Associates Degree in Nursing. Bachelors in Nursing preferred.\n\nExperience\n\nMinimum two (2) years of nursing experience in an acute care setting. Previous home care experience is desirable but not required.\n\nLICENSURE\n\nRegistered Nurse with a valid (or eligible) Connecticut license required. A valid driver's license and proof of car insurance are required when using a personal vehicle for company business. Access to a dependable vehicle in order to travel to multiple stops a day. CPR certification is required and must be maintained. Candidates who do not have certification are required to obtain it within 3 months (90 days) of hire.\n\nSpecial Skills\n\nExcellent clinical knowledge, verbal / written skills, and organizational skills. Must be able to work independently. Able to competently assess patients' needs and follow through accordingly. Strong ability to work with providers and members of the multidisciplinary team.\n\nPHYSICAL DEMAND\n\nAbility to perform tasks involving physical activity, which includes heavy lifting, extensive bending, administering CPR, patient bracing, standing, and stairs.
