Position Summary :

Paramount Home Health Care LLC is seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to provide skilled nursing care to clients requiring G-tube (gastrostomy tube) management. The LPN will ensure safe administration of feedings, medication via G-tube, and monitor overall health conditions while maintaining accurate clinical documentation in compliance with state and agency policies.

9AM TO 5PM And Monday trough Friday. 40 Hours a week .You re a dog friendly !

Key Responsibilities :

• Perform G-tube care and management , including cleaning, maintenance, and ensuring patency of the feeding tube.
• Administer prescribed enteral feedings and medications through G-tube according to physician orders.
• Monitor clients for any adverse reactions or complications such as infection, leakage, or blockage, and report promptly to the supervising RN or physician.
• Conduct vital signs assessments , observe changes in condition, and document all findings accurately.
• Provide education and support to clients and family members on proper G-tube care and feeding techniques.
• Follow infection control procedures and maintain a clean and safe environment at all times.
• Collaborate with the interdisciplinary team to ensure continuity of care and update care plans as needed.
• Maintain timely documentation of nursing interventions, observations, and progress in the client’s medical record.
• Participate in ongoing training and competency evaluations related to G-tube management and other specialized care.
