The Registered Nurse (RN) Case Manager, in partnership with physicians, nursing and healthcare team members, utilize professional skills to assess patient and family for medical, psychosocial, discharge needs and establishes and documents plans for effective management across the continuum of care. The RN / Case Manager role also includes proactive, individualized planning for patients’ progress that optimizes quality of care, patient satisfaction, utilization of resources and reimbursement to meet organizational strategic objectives.

Education Required : Currently licensed or able to become licensed to practice as an RN in Wisconsin, BSN preferred.

Related Years of Experience Required :

Three years’ experience in related field

Previous Care Coordination, Case Management or Home Health experience preferred

Previous experience with mobilizing community resources, navigating patients through the healthcare continuum, and working with disparate populations preferred.

Shift :   Working 40 hours a week, rotating weekends.
