Participation in our on-call rotation is required, with a minimum commitment of one weekend per month or equivalent. - Scheduling and performing prospective new client assessments\n
- Consultative selling to convert leads to clients\n
- Writing and maintaining Service Agreements and Care Plans in compliance with state regulations\n\n2. Caregiver Preparedness\n
- Participating in New Caregiver Orientation, including skills evaluation and training\n
- Briefing caregivers and facilitating client / caregiver introductions\n
- Supervising, supporting and coaching caregivers\n\n3. Quality Assurance\n
- Monitoring shift notes and client conditions\n
- Performing supervisory visits and unannounced quality assurance visits to assess quality of care and client satisfaction\n
- Tracking and reporting incidents, compliments and complaints\n\n4. Community Relations\n\nEstablishing and nurturing relationships with referral sources\n
- Participating in networking events\n
- Contributing to marketing efforts\n
- Additional responsibilities include providing care coordination for clients enrolled in this\n
- standalone service, ensuring regulatory compliance, performing miscellaneous administrative duties and other tasks as assigned.\n\nIdeal candidates will be : \n

- Valid Vermont LPN or Registered Nurse (RN) license\n
- Valid driver's license and reliable transportation\n

- Proficiency with Microsoft (Word, Excel, PowerPoint) and Google (Gmail, Calendar,\n
- Drive) programs\n\nAt least 5 years of nursing experience\n\nPreferred Qualifications / Experience\n
- Experience in non-medical home care, home healthcare and / or home hospice strongly preferred\n
- Clinical and administrative nursing experience\n
