job_description.job_card.job_descriptionWork in a hospice home health setting serving geriatric patients Day shift schedule with 8-hour shifts, Monday through Friday, 7 : 00am to 3 : 00pm No weekend or on-call requirements Typical caseload involves up to six patient visits per day with a 1 : 1 ratio and 30 productivity points standard Mileage reimbursement provided at the IRS standard rate Documentation required using HomeCare HomeBase EMR system Occasional travel up to a 50+ mile radius within Sonoma County Responsibilities include conducting hospice admissions and providing patient care in home settings Timely communication and electronic documentation are expected Documentation tools will be provided and must be returned at end of assignment Scrubs of any color are acceptable Free parking on site First-time travelers are welcome to apply System minimizes driving and windshield time by grouping clients where possible Active CA RN license required BCLS certification required Minimum one year of recent skilled hospice experience within the last three years Experience with geriatric patient population Own transportation is required High Pay and Bonuses Medical, Dental and Vision Insurance with Day 1 Options 401(k) Plan Weekly Payroll Deposit Free Online CEUs Generous Housing Allowance Travel and Licensure Reimbursements Non-taxed Per Diem and Subsidy Traveler Rewards and Discounts
