Description :
The Nurse is responsible for the development, monitoring and evaluation of health related services, policies, procedures, staff In-service training and records which comply with federal, state, agency and service regulations to ensure the health and safety of participants.
DUTIES / RESPONSIBILITIES
- Maintain and / or monitor the maintenance of all necessary records, reports and participant files as related to health needs and services and medications to endure compliance with Board of Health (BPH), Medicaid and Medicaid Waiver regulations for supervised group living and supported living
- Participate in annual Case Conferences and / or staffing, as part of the interdisciplinary team
- Monitor the development and implementation of health related Individual Prescriptive Plan (IPP) goals, objectives and instructional activities established the annual case conference
- Participate in appropriate departmental, agency or other organizational meetings, as needed
- Responsible for returning calls from the pager for emergency or urgent medical situations
- Assist in coordinating participant appointments with the physician
- Conduct visits at each cost center to ensure appropriate medication and health services are provided by direct care staff
- Complete documentation for accident / incident reports for clients
- Complete medication change section on Human Rights Committee reports after psychiatrist appointments or other times of medication change
- Complete all necessary paperwork for purchases made
- Respond to participant emergencies in day services
- Responsible for on call rotation for Residential Services
- Other duties as requested by the Director of Nursing
