Job Description : -

Evaluate, plan, and implement occupational therapy treatment programs.

Provide patient-centered care to promote independence and functional improvement.

Collaborate with interdisciplinary teams to ensure quality outcomes.

Document care in accordance with professional standards and facility guidelines.

Required Skills & Experience :

Active Occupational Therapist (OT) license in MN (or eligibility).

Prior experience in SNF, LTC, Home Health, or Outpatient preferred.

Strong communication and patient-care skills.
