Care Coordinator II

Location : Must reside in Arkansas or a border city

Pay Rate : $23 / hr

Shift : Monday - Friday, 8 : 00 AM - 5 : 00 PM

Dean's Professional Services is actively seeking a Care Coordinator II to support care management activities and ensure continuity of care and member satisfaction. This role works closely with healthcare providers, partners, and members - performing outreach telephonically and through home visits while maintaining accurate care documentation.

Responsibilities

  • Coordinate care activities according to the care / service plan in collaboration with healthcare providers, community partners, and members / caregivers.
  • Serve as a support resource for member and provider inquiries, requests, or concerns related to care plans.
  • Communicate with care managers, practitioners, and others to facilitate member services and ensure continuity of care.
  • Perform service assessments / screenings and document member care needs as required.
  • Maintain accurate member records in compliance with state and regulatory guidelines.
  • Identify needs and make referrals to Care Managers, Disease Managers, or community-based organizations.
  • Provide education on benefits and available resources.
  • Perform other related duties as assigned.

Qualifications

  • High School diploma or GED required.
  • 1-2 years of related experience preferred.
  • Valid driver's license required.
  • Travel : Up to 30% required.
